To login:

(1) type in the EMAIL you used to create your appointment;

(2) type in the PASSWORD that was sent to you from Vitality. Be sure to check your spam/junk folder for an email from us.

*If you don’t find an email with a password sent to you, call 843-757-2270 to notify our office.

After logging in, you can also change your password by clicking the “Change Password” tab on the right lower screen.

Once logged in, you are able to fill out your paperwork prior to your visit, make appointments, view your chart, fill out forms, message providers (this is how you can cancel appointments as you cannot cancel online) and pay any outstanding balances.
